Responsibilities include following the plan of care, providing treatments, and working collaboratively with the members of the team to help meet positive patient care outcomes.

Job Title: Licensed Practical Nurse (LPN)

Essential Job Functions/Responsibilities

  1. Provides direct patient care as defined in the state Nurse Practice Act.
  2. Implements plan of care initiated by the registered nurse.
  3. Provides accurate and timely documentation consistent with the plan of care.
  4. Assesses and provides patient/family/caregiver education and information pertinent to diagnosis and plan of care.
  5. Participates in coordination of home health services, appropriately reporting the identified needs for other disciplines (HHA, OT, PT, MSW, ST, Dietician) to the registered nurse and/or Clinical Supervisor.
  6. Uses equipment and supplies effectively and efficiently.
  7. Participates in personal and professional growth and development.
  8. Performs other duties as assigned by the registered nurse.

Position Qualifications

  1. Graduate of an accredited practical nurse or vocational nursing program.
  2. Has three (3) years nursing experience. Community/home health or medical/surgical experience is preferred.
  3. Currently licensed as a licensed practical nurse or licensed vocational nurse in the state
  4. Complies with accepted professional standards and practice.
  5. Demonstrates good verbal and written communication, and organization skills.
  6. Possesses and maintains CPR certification.
  7. Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.

Physical Requirements:

  1. Body Positions: Sitting for prolonged periods.
  2. Body Movements: Arm and hand dexterity
  3. Body Senses: Must have command of close and distant sight, color perception, and hearing.
  4. Strength: Ability to lift and move up to 25 pounds.

Working Environment:

  1. Time spent working in an office, where the climate is controlled. Travel to various local offices, i.e., physician’s office, hospital for meetings.
  2. Travel by car to patient’s homes, where the employee does not have control over how the climate is regulated.
  3. *OSHA Exposure Category:  II

OSHA Category I – Position includes tasks that involve exposure to Bloodborne Pathogens.

* OSHA Category II – Position includes tasks that do not have exposure to Bloodborne Pathogens; however, employment may require unplanned Category I tasks

OSHA Category III – Position includes tasks that do not involve exposure to Bloodborne Pathogens. This position would not be required to perform Category I tasks.
