Adult Services

At Pennsylvania Hope Home Health Care we are proud to serve medically fragile patients. This experience means that we′ re familiar with navigating the home care process and we know that the most important things are  staying healthy and staying home with family. The services that our healthcare professionals provide allow our patients to be at home instead of a long-term stay in a hospital or care facility.

We will work with you, your family and your physician to develop a care plan that meets your needs. To make the process even easier we accept multiple payer sources including Medicaid, Medicare, waiver programs, commercial insurance and private pay.

Aide Services

In-home care for patients who require assistance with activities of daily living, Pennsylvania Hope Home Health Care provides aides to assist with or provide care as outlined and developed by a supervising RN in the Aide Plan of Care.

Private Duty Nursing

In-home nursing care for adults with high-tech needs tracheostomies and feeding tubes. Pennsylvania Hope Home Health Care RNs and LPNs/LVNs can provide up to 24 hours of care each day, 7 days a week depending on individual needs and physician orders. An individual care plan is developed for each patient and carried out by skilled member of our nursing staff.

Respite Nursing

Sometimes the responsibilities of being a caregiver overshadow other aspects of your life. If you need some time to care for yourself or other family members, reduce stress, catch up on rest or other activities Pennsylvania Hope can provide respite care during these times. Respite care allows you to be a caregiver for your family member, but also receive much-needed support when you need it.

Skilled Nursing Visits

Unlike Private Duty Nursing, a Skilled Nursing Visit is not a full nursing shift. An RN or LPN/LVN will come to the home to administer medications, clean dressings and check certain vital signs, along with other similar nursing procedures. Depending on the care needed and the physician orders, a patient may receive multiple visits a day or just a few each week.

What Services We Provide:

We are available 24 hours a day, 7 days a week, including public holidays our experienced team of professionals provide expert medical care including∶

In-Home Assessments:

  • Non-Medical Assessments are conducted to find clients’ functioning capabilities, to identify their needs, and to determine what services are required to assist them attaining and maintaining optimum independence.
  • Nursing Assessments are an expanded version of the non-medical assessment as they are more thoroughly examine and identify health conditions, levels of care required and appropriate medical interventions, if indicated, coordination of care, with other health professionals can be arranged.

In both situations, assessors may review subsidization possibilities, develop care plans, make referrals, and arrange for service delivery schedules according to the clients’ wishes.

Home Services

  • Planning, cooking and serving meals
  • Monitoring and motivating regular diet plans
  • Transporting clients to social activities and appointments
  • Running errands and shopping for clients or transporting clients to carry out these activities
  • Providing respite care
  • Serving as companions and participating in friendly socialization activities
  • Assisting with entertainment preparations, hosting, serving and/or cleanup
  • Caring for pets including feeding, exercising, cleaning litter boxes, cleaning birdcages, taking to grooming appointments or to the vet
  • Caring for plants
  • Monitoring clients’ safety, comfort and welfare by contacting them by phone or by making home visits

Personal Care:

  • Assisting, with grooming needs such as hair care, shaving, cleaning teeth and routine nail care
  • Supervising or assisting with tub baths, showers, sponge baths and bed baths
  • Providing routine skin care such as applying lotion to dry skin
  • Assisting with dressing and undressing
  • Assisting with mobilization activities including turning, transferring, exercising and walking
  • Assisting with toilet and/or commode needs
  • Assisting with eating or feeding
  • Assisting with self-administered medications by reminding client to take them at the prescribed time(s) and/or following up to ensure they are taken
  • Monitoring vital signs such as blood pressures, pulse and respiration
  • Providing respite care

24-Hour/Live-In Assistance

For our clients whose circumstances require 24-hour, live-in care, we will typically provide our most experienced and qualified caregivers. There are two different options for live-in care.

  • The first option is the most common whereby a caregiver spends 24-hours in your household with a full 8-hour overnight sleeping break and intermittent breaks throughout the day. The caregiver is available on an emergency basis during their sleeping break.
  • The second option provides “awake and alert” care around the clock whereby a caregiver is available at all hours (with the exception of standard breaks). This care is typically handled in shifts of two (sometimes three) caregivers for clients that need frequent and constant attention throughout all hours of the day.

Benefits of Live-in Care∶

For nearly the same cost as a nursing home, a client receives one-on-one care from a devoted caregiver and is able to remain in their own home.

As part of our care management services, we work with your loved one’s healthcare providers by:

  • Attending doctors appointments and taking notes for you and your loved one
  • Reminding your family member to ask questions or express concerns
  • Providing information your senior parent has forgotten, or clarifying information for the doctor
  • Acting as a “translator” who explains what the doctor and patient are saying to each other
  • Helping your elder to make the necessary follow-up diagnostic tests and appointments, with doctors and therapists
  • Offering friendly reminders to take medication as prescribed or follow other doctor’s orders
